    source)
    4. Copy of your driver’s license or passport. If non-US Citizen, we need copy of current VISA.
    5. Proof of income (either a current pay stub or current offer letter) for job and pay verification.
    If you are self-employed and not taking regular pay I will need two years of your page 1 1040
    Federal Tax Return from the past two years for pay verification.
